You have choice and control over your care.

The Alaska Veteran Directed Services program offers enrolled Veterans more choice, control, and flexibility over the services they receive. The program includes participant-directed services as an alternative to the VA’s agency-based care options.

Eligibility

The Alaska Veteran Directed Care (VDC) program offers enrolled Veterans more choice, control, and flexibility over the services they receive. The program includes participant-directed services as an alternative to the Veteran Affairs (VA) agency-based care options.

Veterans may qualify for directed care if they meet the following criteria:

  • Enrolled in the Alaska VA Healthcare System
  • Have a Primary Care Provider
  • Can self-direct their own care
  • Meet clinical criteria
Get Started

Veterans in Alaska in need of long-term care services can access the Veteran Directed Care (VDC) 907-776-2137 Program through the Alaska Veterans Administration (VA). The VDC program goals are to facilitate Veteran’s choice & autonomy- allowing them to remain independent in their homes & community.

  • If a community provider believes that a Veteran needs home health care (not hospice), providers should submit a Request for Services (RFS) to get care authorized. VA typically has up to 14 days to review and approve this request.
  • For Veterans being discharged from inpatient care, the ordering physician or the facility must submit an URGENT RFS to the VAMC prior to discharge.
  • All care must be pre-authorized.

Fill out this form with your primary care provider and return the completed form to the VA.
